Add date/time info in the UI for blocked IP addresses

In the Notes column for the IP Manager/ IP Deny screen, it would be really nice to include the date and time in the UI that the IP was blocked rather than having to go comb through log files to figure it out.  Including any identifying information in addition to the date and time would be nice as well.  While I do understand that this is included in emails alerts, we sometimes become numb to the alerts because we get so many of them.
